Insurance & Financing Options

We believe
in making financial arrangements
as convenient as possible in our office.

 Please call our office to discuss specific financing options if you are in need of one.

Most people are concerned about the costs of dental services and whether or not we accept their insurance. We will gladly review your benefits with you and help you to get your needed treatment.

Financing Options:

We offer convenient financing options from

  • Dental Fee Plan which range from three months with no finance charges to up to five years financing depending of the amount needed.
  • Care Credit Program -  Low monthly payments to fit comfortably into almost every budget.  click here for more information about Care Credit.

Credit Options: We accept Visa, MasterCard, Discover and Amex.

Insurance Options: Insurance companies often confuse patients about their coverage amounts and benefits. We will help you to understand them. Most insurance covers 100% of the cost to evaluate your dental health. This includes X-Rays, exams, and routine cleaning. We accept all traditional indemnity plans and many Preferred Provider plans. We do not accept capitation plans.

Some of the plans accepted are: - Aetna - Blue Cross - Blue Shield - Cigna - Guardian - Delta - Dental Benefit Provider (DBP) - Dental Health Alliance - Humana Dental - Metlife - Principal - Prudential - United Concordi - Ameritas

Insurance Definitions: Deductibles This is the portion for which you are responsible to pay of the first filed claim. Most commonly the deductible does not apply to preventive services.

Maximum
This is the amount your insurance company will pay each year.

Preventive or Class I
This includes X-Rays, exams and routine cleaning.

Restorative or Class II
This refers to fillings, root canals, extractions and periodontal work.

Major or Class III
This refers to crowns, bridges, dentures and partials

Policies

Appointments:

Our doctor and staff value your time. In order to provide excellent care to all our patients, we request that all appointment cancellations be made at least 24 hours in advance.

We also respectfully reserve the right to reschedule any patient who is fifteen minutes or more late for their scheduled time. This ensures that you’ll avoid being billed for our time and preparation; it also enables us to work on other patients’ emergency visits.

We reserve the right to charge a fee for missed appointments or appointments cancelled less than 24 hours in advance.

Emergencies

For after hour emergencies, please call the office for instructions on how to page the doctor.

Payment

Payment is due in full at the time of treatment unless prior arrangements have been approved.

Please note that  treatment plans are estimates only.  X-rays and examinations do not always reveal the full extent of existing conditions.  A more extensive treatment may be needed, resulting in unanticipated charges.  The estimated amount of insurance coverage may also vary due to conditions or specifications of your policy which we are not aware of.

Below are  payment options:

1. Payment of all charges in full can be paid in Cash, Check  or Credit Card (10% Adj)    Does not apply if insurance is involved

2. DFP Program or Care Credit Program (approval required)

3. Payment Plan max 3 payments (1.5% [18% APR], interest after 90 days)

4. Chek plus ____ day of month ____ payments.  (6 - 10 ) payments only.

5. Credit Card Approved

NOTE: As a service from our office, we will bill your insurance ONE TIME as soon as services are provided.  Patient is responsible for amount not paid from insurance within 30 days of service.
